MID-PROGRAM EVALUATION
Mentee & Mentor	Complete the OCA Mid-Program Evaluation and submit to OCA three months into the program. Your evaluation form will be sent to you via email. Please watch your inbox.
END OF THE PROGRAM
Mentee & Mentor	Complete Mentee & Mentor final evaluation forms and submit to OCA. Your evaluation form will be sent to you via email. Here are a few helpful guidelines for your final meeting Upon conclusion of the program, obtain CE credits and a Certificate of Completion (participants will receive this upon meeting the terms of the program, please refer to the Application Agreement for further details)
