Opportunity Overview
Hamilton Health Sciences is seeking community-based chiropractors and physiotherapists in the Niagara Region to participate as Advanced Practice Providers (APPs) in the Rapid Access Clinic for Low Back Pain (RAC-LBP) program.
This is a unique opportunity for clinicians practicing in the community to take on an expanded, specialized role within a provincially funded spine care pathway, while maintaining their independent practice.
APPs deliver standardized assessment and care planning for low back pain as part of an integrated regional network, helping ensure patients receive timely, appropriate care close to home.
About the RAC-LBP Model
- A Ministry of Health–funded program improving access to low back pain assessment across Ontario
- Assessment-focused model (not ongoing treatment) delivered in 1–2 visits
- Patients referred by primary care providers and seen within ~4 weeks
- APPs provide:
- Comprehensive, standardized spine assessments
- Risk stratification and triage
- Evidence-based education and self-management plans
- Patients are redirected appropriately to primary care, imaging, or spine specialists as needed
Role of the Community-Based APP
As a RAC-LBP APP, you will:
- Conduct standardized low back pain assessments within your community clinic setting
- Deliver evidence-based education and self-management strategies
- Provide clear consult reports and recommendations to referring providers
- Identify patients requiring escalation to imaging or surgical consultation
- Use the provincial RAC platform for documentation and reporting
- Participate in ongoing training, mentorship, and quality improvement initiatives
- Collaborate within a regional hub-and-spoke model connected to Hamilton Health Sciences
Who Should Apply
We are looking for Niagara Region–based clinicians who:
- Are registered and in good standing with:
- The College of Chiropractors of Ontario, or the College of Physiotherapists of Ontario
- Have strong expertise in Musculoskeletal and spine assessment
- Demonstrate excellent clinical reasoning and communication skills
- Are committed to evidence-based, patient-centered care
- Are interested in working in a consultative, triage-focused role (not ongoing therapy)
Preferred:
- Experience with interdisciplinary or triage-based models of care
- At least 5 years of direct patient care
- Interest in system-level impact and reducing unnecessary imaging and referrals
- Familiarity with RAC-LBP / ISAEC model (training will be provided)
